Golden Valley Memorial Healthcare (GVMH) is pleased to announce the launch of its 2025 Community Health Needs Assessment (CHNA) survey. The survey is now available online and open to all community members through May 31, 2025.
The CHNA is a critical tool that helps GVMH understand the most pressing health needs facing the communities we serve. Through this survey, residents have the opportunity to share their perspectives on local health concerns, access to care, and services needed to improve overall health and well-being.
“Community input is essential to shaping our healthcare programs and services,” said Rachel Boyles, Chief Ancillary Officer at GVMH. “The CHNA helps us connect research, resources, and people to ensure we are aligning our efforts with the true needs of our community.”
The CHNA process includes gathering both data and personal insights through surveys, interviews, and input from public health leaders, healthcare professionals, and community partners. The results will guide GVMH in developing an implementation strategy to address the identified needs through new initiatives or enhancements to existing programs.
